End of life care, or advanced directives, is something that's difficult for people to talk about.

If you want help getting your advance directives in order, experts say to talk to your loved ones about how you'd like to be cared for at the end of your life.

Find a friend or family member comfortable making those decisions with you and who will carry out what you want.

Revisit those decisions over time, because sometimes our values can change.

Experts say it's also important to pick someone to speak for you and make decisions when you can't.

"It should be someone who you have asked, so they're in agreement with taking on this role," explained Sandy Babic with University Hospitals. "Because it's a weighty job, and you should make sure they speak with your voice."
