Swine Flu Vaccine Locations Announced For Families

LAFAYETTE _ The state Department of Health and Hospitals will provide H1N1 2009 flu vaccines to
Lafayette Parish students on December 5, 2009. The Lafayette Parish School System has partnered with state health officials to open its schools for the vaccine clinics.

This week, students were given a release form and information packet to take home to their parents. Parents should read the information and fill out the release form, and take the release with them to the clinic on December 5 between 8 a.m. and 3 p.m. Parents who did not receive the packet can download it online at www.lpssonline.com/h1n1info.

LPSS staff will be assisting state health officials in taking histories, but DHH staff will be administering the vaccine in nasal and injection form. Families can start lining up at 7:30 a.m. but no earlier. Anyone who is not in line at 3 p.m. will not be seen.

Parents of children who have asthma, other respiratory conditions or any chronic health condition should consult with their physician prior to arriving at the clinic. Children with asthma and/or respiratory conditions will not receive the nasal vaccine. The vaccine is for children in grades K through 12 only. People in other age groups, even those in the identified risk groups, should contact their physician or the health unit for vaccine.

There are five sites where the clinics will be offered. Families should go to the site assigned to their home zone.

Families zoned for Acadiana High School will receive their vaccines at Scott Middle School.
Families zones for Carencro High School will receive their vaccines at Carencro Middle School.
Families zoned for Comeaux High School will receive their vaccines at Plantation Elementary School.
Families zoned for Lafayette High School will receive their vaccines at Lafayette Middle School.
Families zoned for Northside High School will receive their vaccines at the N.P. Moss Annex (old N.P.
Moss, at the corner of Mudd and Moss streets).
